2. Take all measurements and outlines over normal socks and jeans while seated.

3. For each foot, take 12 measurements at the points indicated by the lines in the diagram.

4. Fill your height and weight.

5. Indicate whether your measurements are in inches or centimeters.

6. Indicate the color of choice.

7. Indicate whether the boots are Triple Layer Boots or D3O Boots.

8. Fill in your initials if needed.
